Healthcare Delivery Capacity Building VISTA
 
 
 
NYC Department of Homeless Services Office of the Medical Director (OMD) works to improve the health of individuals experiencing homelessness in NYC to increase their chances of succeeding in permanent housing. We accomplish this goal through a variety of evidence-based programs that are rigorously tested, evaluated, and adapted to meet the unique needs of people experiencing homelessness. OMD partners with a variety of organizations to achieve our mutual goal of improving the health of clients experiencing homelessness. The 2022-2023 VISTA member will be instrumental in addressing health inequity among individuals experiencing homelessness. OMD works closely with NYC hospitals and shelter providers to improve care and implement policies to ensure clients health needs are met. A VISTA member is crucial to expand our capacity to implement and sustain programs by developing trainings and resources to support improved service delivery. This position is ideal for a candidate interested in working with vulnerable populations and collaborating with NYC health providers. The member would be part of an innovative team that takes a public health approach to addressing challenges faced by people experiencing homelessness. This VISTA position is a part of the NYC VISTA program, a group of 25+ VISTA members serving in New York City Government agencies supporting their anti-poverty work. The NYC VISTA program is administered by NYC Service, a division of the Office of the Mayor. This position is supervised directly by a city agency staff member and will be a part of the larger NYC VISTA community. Member Duties : If chosen, the VISTA will mainly work in 3 main capacities which are as follows: (1) Expand Department of Homeless Services capacity to review and monitor referrals from healthcare facilities and prisons for new patients being referred to shelter and ensure that they are medically appropriate for shelter. (2) Provide shelter staff with the necessary tools to assist clients in accessing appropriate medical and behavioral health care, including submitting Single Point of Access (SPOA) referrals for Intensive Mobile Treatment (IMT) and Assertive Community Treatment (ACT) services, and requesting medical consultations when necessary to escalate issues. (3) Work with supervisor to develop standard methods and analytic plan for identifying the biannual High-Risk Shelter Client List
