PCCE- Juniata College (Preventive Health)
 
 
 
*Applicant will need to provide Covid-19 vaccine documentation* Juniata College, a small, private liberal arts college, is situated in rural Huntingdon, Pennsylvania, which is a safe and generous community surrounded by opportunities for outdoor recreation. Juniata College is less than an hour’s drive from State College and Altoona. We plan to work with Broad Top Area Medical Center (BTAMC) to implement a community health initiative that will assist patients with psychoeducation regarding diseases and care while providing resources to improve their quality of care. BTAMC offers limited case management services which we aim to strengthen through this program. We aim to provide a more integrative approach to health by including a focus on physical health, dental health, and behavioral health. For those living in poverty, these aspects of health are difficult to prioritize and are often unattainable for various SDOH factors. The VISTA would aid in developing partnerships between Juniata, BTAMC, and other community stakeholders, particularly those related to workforce development. The program will also allow Juniata students to assist providers and patients to ensure their needs in all aspects of their care are being met.

Member Duties : "The VISTA will begin year 2 of the Preventive Health project. The overarching goal is to assist in the development and delivery of health programs that reflect best practices and evidence-based care. The VISTA will perform the following tasks and activities: Perform a community assessment; Building community awareness and engagement; Expand and strengthen partnerships; Material development; Outreach; Performance measurement; and Program development and delivery. The VISTA should be a leader with strong interpersonal and organizational skills. The VISTA will also be expected to have competencies in research and technology, as well as strong written and oral communication skills and knowledge of and/or experience with project management. The ideal candidate will be a problem-solver who is resilient and can tolerate ambiguity.
