TSC EOC 2021: Evergreen Health Services
 
 
 
The EOC member would specifically be trained in housing navigation services and SSI/SSDI Outreach, Access, and Recovery (SOAR) programming, which works towards assisting eligible individuals with securing SSI/SSDI benefits. This position would allow our housing programs to expand and grow positively as SOAR representatives play a large role in stabilizing patients' incomes. The EOC member would be piloting a SOAR program for Evergreen Health, they would begin with a small caseload during their term. To ensure we can serve as many people as possible, the EOC member would also assist with housing navigation for individuals who need assistance with finding and securing safe and affordable housing. They would assist with assessing a patient's financial stability, create a plan towards stabilizing it, and assist with housing navigation (finding safe/affordable housing). The added benefit of completing this type of housing work would be the member's ability to identify individuals who may be eligible for SOAR and then have the ability to immediately start working with them on the next step. We currently have a full-time team member supported by HOPWA funding who assists patients with housing navigation (hereinafter HN) and assists 180 patients a year with finding safe and affordable housing, linkages to landlords, financial assessments, and referrals to other resources. The EOC member would assist the current Housing Retention Counselor who completes these services. The member would work with the patient on housing navigation and stabilization. The member would then end services with the patient once housing is secured. The EOC member would be working with people who are homeless, at-risk of homelessness and living with a chronic condition (i.e. mental health, HIV). The member would have the opportunity to work with many other populations including DV survivors, veterans, and families throughout Erie, Niagara, and Chautauqua counties.

Member Duties : The AmeriCorps member will work towards providing SOAR services to people already enrolled in our housing programs. The member will work with the patient on obtaining SSI/SSDI benefits in under 90 days, which will then increase their income and achieve their goal of financial stability. Through these or other financial stabilization services, the member will also assist those who need immediate housing navigation assistance by searching for and securing safe and affordable housing for said patient. They will connect with landlords, assess apartments, assist patients with understanding their lease and tenant rights, and provide financial assistance in the form of first month's rent and security deposits.
