Health Alliance Project Coordinator
 
 
 
To help ensure that residents of West Mountain (Eagle, Garfield, and Pitkin counties) break the cycle of poverty and receive integrated health care and support around Social Determinants of Health (SDoH)*. This will promote the effort to build an equitable health system for all. You will build upon the region’s health system and support public health and equity system efforts by working with the Community Resource Network to ensure all SDoH resources and guides are accessible, accurate, and updated on a quarterly basis. The VISTA will build the capacity of the West Mountain Regional Health Alliance by helping to develop and maintain a regional data dashboard that will serve WMRHA partners and the broader community. The VISTA will additionally support fundraising efforts to provide financial and capacity resources to meet data identified gaps. *Social determinants of health (SDOH) are the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks.

Member Duties : Build the capacity and reach of the Community Resource Network (CRN) by working with existing and recruiting new system level partners. Build and augment the WMRHA Data Dashboard with the WMRHA Logic Model and shared data sources to provide information and measure shared outcomes. Take part in fundraising, grant research and grant writing.
