Maternal & Infant Health Innovation - SE Ohio Foodbank

The Southeast (SE) Ohio Foodbank, a HAPCAP (Hocking Athens Perry County Action Program) program, is a dynamic nonprofit organization that works to end hunger in SE Ohio. We mobilize food, support and resources to create a lifeline for people in need and to increase community access to healthy food. We are a distribution center and community hub that collects and distributes government-funded, donated and purchased food to our member agencies in our 10 county service area. Our network of member agencies operate programs such as food pantries, hot meal sites, shelters and other hunger relief programs. Our regional kitchen prepares food for Meals on Wheels, Senior Congregate Dining, Head Start Meals and Summer Meals. Additionally, we have a Health Equity team of Community Health Workers who work to address the Social Determinants of Health, by employing Food as Medicine initiatives as a Care Coordination Agency in partnership with the COAD Community Pathways HUB, giving priority to serve individuals who are pregnant, new parents or live with chronic health conditions.
