OHA Health Share Health and Housing VISTA
 
 
 
This position joins the Oregon Health Authority’s statewide VISTA team. In addition to activities at your individual site, each member receives additional orientation, training throughout the year, team retreats and meetings, and the support of the team leaders and project director based at the state public health agency. Selections are made on a rolling basis and early applications are strongly encouraged. Health Share of Oregon serves approximately 380,000 Oregon Health Plan members in Clackamas, Multnomah, and Washington counties. We bring together health plans, providers, and community health resources so our members can access the care they need to be well. The Oregon Health Plan (OHP) is our state’s Medicaid program. It provides no-cost health coverage—including medical, dental, and mental health and substance use benefits—to individuals and families who qualify. In addition to coordinating care and resources for members, we work with community partners in health care, education, housing, transportation, and social services to broaden health access and opportunity. Together with our partners, we are creating community-based solutions that lower everyday barriers to health, like providing rides to health care appointments, offering opportunities for peer support, and more. The OHA VISTA Partnership Project is committed to building a diverse workforce to broaden and deepen our work in the communities we serve. Member Duties : The Health and Housing AmeriCorps member will support Health Share’s portfolio of initiatives. These initiatives seek to increase access to transitional and supportive housing for our Coordinated Care Organization’s (CCO) Medicaid members in order to address this critical social determinant of health. The member will conduct an environmental scan of housing-related projects underway within health care settings and work to support workflows between health and housing providers. They will gather information from in-person interviews, online sources, and by reading relevant public documents and document processes to support access to housing for OHP members transitioning from institutional settings. The member will plan a half or full day convening where they will present this information to key stakeholders with the goal of informing Health Share’s strategic efforts to increase access to transitional and supportive housing for our members.
