OHA Accountable Health Communities VISTA
 
 
 
This position joins the Oregon Health Authority’s statewide VISTA team. In addition to activities at your individual site, each member receives additional orientation, training throughout the year, team retreats and meetings, and the support of the team leaders and project director based at the state public health agency. Selections are made on a rolling basis and early applications are strongly encouraged. Many of ORPRN’s projects are focused on improving health equity for Medicaid and Medicare populations in rural areas. We work with the Oregon Health Authority and Coordinated Care Organizations to increase access for these populations to preventive care, including cancer screening and immunizations. We manage a community benefit program that provides direct grants to community organizations to alleviate homelessness and hunger. We also provide technical assistance and education to clinicians and staff of Coordinated Care Organizations to improve access and quality of care for Medicaid and Medicare populations. Portland is known for the legendary Powell’s Bookstore, its culinary scene, and microbrew industry. The Pacific Ocean, Mt. Hood and acres of national forest are within one to two hours’ drive, and within city limits you can find 7,500 acres of park land, including Forest Park, the largest urban wilderness within any US city. Portland has excellent public transportation and is recognized nationally as a bicycle friendly city. With the innovative public health work happening in Oregon, Portland is a wonderful place to explore a future in public health. Member Duties : The Accountable Health Communities [AHC] project seeks to develop integrated, voluntary systems across the health care, public health, and social service sectors to identify the social needs of medically underserved populations in rural areas and to assist these populations with accessing services to resolve their needs. The AmeriCorps member will increase understanding of and participation in the AHC model by social service agencies. Member activities include: 1) Identifying shortcomings in the services available to Medicaid and Medicare patients, including unmet needs in housing, transportation, utilities, safety, and food security; 2) Creating data visualizations and reporting findings to social service agencies and project leadership to foster inter-organizational collaboration; and 3) Providing education and support to Coordinated Care Organizations in the implementation of three projects to support Medicaid beneficiaries in meeting unmet social and health needs.
