Member Duties : Primary Responsibilities
1. Assess tobacco addiction level and nicotine withdrawal
2. Recommend medications to manage withdrawal while patients are in the hospital
3. Provide brief counseling and treatment planning
4. Collaborate with medical team to provide discharge medications
5. Proactively refer patients for follow-up support post hospitalization
6. Document encounters in the electronic medical record
7. Contact patients one month post discharge to conduct a brief evaluation survey
8. Participate in process improvement activities to enhance the delivery of services
