*Applicant will need to provide Covid-19 vaccine documentation* The Institute for Nonprofit Leadership builds civil society through a vibrant nonprofit sector. Our mission is to provide individuals and organizations with the education, research, services, and networks necessary to understand, strengthen and advance the work of the nonprofit sector Healthy Heart, Health Futures programs, through the BHS LifeStyle Medicine Institute (LSMI), are designed to help low-income individuals more effectively manage the social determinants of their chronic heart disease, leading to a higher quality of life, reduced health care costs, and greater disposable income for other needs such as housing, education, food. Healthy Hearts, Healthy Futures full-time staff, with participant input, will construct a comprehensive, individualized treatment plan that refers a patient to relevant BHS clinica support services (nutritionist/dietician, Diabetic Support Group, Healthy Foods Center, licensed counselors, etc) as well as to community-based programs, such as food banks, housing services, child care options, job training and other initiatives that would help the patient manage their various social determinants of health. VISTA Responsibility: Using a collective impact approach, the Institute will create systems that strengthen networks among BHS medical support services, community-based non-medical support, and SRU to ensure a seamless continuum of care that enhances the quality of life of those participating in the Healthy Hearts, Healthy Futures program. In general, the VISTA will 1) catalog available existing non-medical support services and identify gaps in delivery for individuals with chronic heart disease, 2) leverage community and SRU support to address the gaps facilitate, and 3) design and manage a repository for Healthy Hearts, Healthy Futures program information, data, reports, etc.

Member Duties : Using a collective impact approach, the VISTA will create systems that strengthen networks among Butler Health System (BHS) medical support services, community-based non-medical support, and SRU faculty/students to promote a countywide culture that supports social determinants of health in order to ensure a seamless continuum of care that enhances the quality of life, low-income individuals.
