CoF FHP Project Development Specialist VISTA
 
 
 
Fairbanks Memorial Hospital is the sole community hospital serving interior Alaska. The organization conducted a Community Health Needs Assessment (CHNA) in 2019 to identify unmet health needs of the area. In 2020, teams were created to collaborate on the Community Health Improvement Plan (CHIP) and meet priorities defined in the CHNA. The VISTA will support development and success of the community partner work teams. There are 4 CHIP focus areas, with a team for each. They are mental health care, senior care, Adverse Childhood Experiences/Trauma, and healthy behaviors. A 5th focus, responding to Covid-19 was added, and each prior team now includes impact and outcomes of Covid-19. Each team aims to improve resources, services, and access to care; particularly for those who are underserved, have unmet needs, and/or who are in poverty or at risk for poverty. Project work includes outreach, managing systems for team communication and process, conducting research, analyzing resources, gathering and reporting data, defining metrics, and evaluating and summarizing best practice models. Project work will focus on senior care and mental health teams. The senior population of interior Alaska has too few affordable safe housing and assisted living options. Similar needs exist for those with mental health needs who are homeless or at risk for homelessness. Each of these teams is focused on building capacity for independent living and supportive services. The project will establish advocacy and awareness efforts, evaluate funding opportunities, and research model programs. The objectives of the role are to 1) understand the FHP CHNA and CHIP, 2) support and increase collaboration of each work team, 3) build capacity of the senior care team focused on low income independent and assisted living, 4) build capacity of the mental health team focused on safe housing for low income individuals with mental health needs.

Member Duties : Orient to the hospital CHNA and CHIP project by reviewing standards required of non-profit hospitals, FHP plans + documents, and health factors + outcome measures for the service area. Support activities of the teams for the 4 CHIP priorities by organizing communication tools, assisting w/tasks that advance team tactics, developing dashboards for improvement mapping, + evaluating teams’ progress. Communicate the work of CHIP teams by drafting web page + social media content. Assist senior care + other teams w/advocacy/awareness campaigns by planning events + platforms incl print campaigns. Interact w/ internal + external partners in the community health network. Research solutions for home + community based services for low-income seniors. Provide teams w/summary resources for models + organize and evaluate input. Investigate funding opportunities + reqs for the senior care team + other teams for model development + promotion especially w/focus on equitable community based services.
