Sunset Park AmeriCorps - Community Health Navigator
 
 
 
Sunset Park AmeriCorps, is a full-time, service program for people ages 18+. Must be a U.S. citizen or permanent resident. Members work directly with the community based programs through the Family Health Centers at NYU Langone. Our Community Medicine Program (CMP), cares for more than 6,000 New Yorkers annually across 14 drop-in centers, shelters, and Single-Room Occupancy (SRO) hotels throughout the city. Our interdisciplinary teams provide comprehensive primary care, behavioral health services, health screenings, outreach, case management, crisis intervention, and long-term counseling, both in person and through telemedicine. CMP health teams give direct medical and behavioral health services, outreach, case management and health screenings, health education, HIV services, crisis intervention and long-term counseling, substance use disorder treatment, performed on-site and on a regular basis, to homeless and marginally housed men and women at 14 homeless shelters, drop-in centers and (SRO) hotels. The communities that CMP serves are diverse and the patient population is often medically underserved, facing many health disparities and barriers to care. Patients may have co-occurring chronic health conditions such as diabetes, behavioral health, substance use, HIV or are at risk for developing these illnesses. Full time NYU Langone Sunset Park AmeriCorps members receive a living Allowance of $23,000, as well an Education Award of $7,395.

Member Duties : • Participate in designing and implementing projects that complement or fill gaps in service in areas of medical, behavioral or substance use disorder treatment. • Conduct outreach to homeless adults (in person or virtually) to assist with accessing medical and behavioral health providers, support programs, and social services • Learn and utilize Epic, (electronic medical record system) to identify patients who would benefit from these services, as well as those in need of exams or screenings and help arrange for the patient’s care and follow through. ‘ • Assist with coordination of services • Act as a patient advocate and resource, • Provide educational resources and develop a plan to fulfill identified needs and overcome barriers in conjunction with the healthcare team and patient.
