The major duties and responsibilities of the Pediatric Public Health Family Navigator is to advocate for quality medical care for individuals who receive services through Trinity Child Care Center. In addition, the individual in this position will advocate for meaningful interactions and opportunities within the community. This is done through the preparation for, attendance at and appointment, attending and providing accurate information to the medical provider and then relaying this information back to the appropriate parties at the Trinity. The Navigator is responsible for assisting in care coordination and resource navigation.

Member Duties : The major duties and responsibilities of the Pediatric Public Health Family Navigator is to advocate for quality medical care for individuals who receive services through Trinity Child Care Center and meaningful interactions and opportunities within the community. This is done through the preparation for, attendance at and appointment, attending and providing accurate information to the medical provider and then relaying this information back to the appropriate parties at Trinity. In addition, the Navigator is responsible for assisting in care coordination and resource navigation. The Health Navigator will also explore meaningful opportunities to improve or expand individual’s mental, social and emotional health by investigating resources within the community that enhance an individual’s quality of life. Maintaining current information about available resources would assist current and future children, their families and the staff who interact with them.
