College Health Corps VISTA, HealthFinders (MNCC)
 
 
 
HealthFinders Collaborative (HFC), a free community health center, provides a comprehensive first access point to health services for the marginalized families of Rice County. Through primary care, medication assistance, patient education and advocacy, and community-based wellness programming, HFC works across the continuum of wellness to engage communities in their own health. This AmeriCorps VISTA position, hosted by Minnesota Campus Compact and HFC, will work to build organization capacity at this critical stage of growth in response to increasing demand for services. The VISTA will do direct community organizing, continuing to facilitate the growing Patient Engagement and Community Leadership Initiative aimed at authentically increasing patient voice in clinic programs, leadership, and administration. HFC believes in the inherent power of communities to care for their own health. As such, we organize culture-specific Family Health Councils to give voice and genuine leadership in decision-making to our patient communities, important work the VISTA will work to deepen and sustain. Additionally, the VISTA will work to mature existing collaborations with two undergraduate institutions that have a long and growing history of collaboration with HFC; the VISTA well as assist in developing the 2013-2014 academic-civic projects, as well as connect and support over two-dozen interns to community wellness programs.

Member Duties : • direct community leadership and involvement initiative for participant voice in program design • facilitate community leaders in discussion and goal-setting, develop trainings • leverage Family councils and HFC staff to contribute to Health Care Home activities and care coordination across all HFC services • deepen collaboration with two local colleges; design unified project(s) for multiple academic departments/students over academic year; oversee college projects and volunteers; convene student leadership meetings across all wellness programs • design ways to meet interest in college community increased demand for services in community; present findings • Create and post bi-weekly newsreel postings on HFC website; direct maintenance of wellness program online bulletin boards and posts. Interviews will occur on a rolling basis and preference will be given to candidates who apply before April 12. Candidates must also email a resume and cover letter.
