Montana’s Community Health Centers (CHC) served more than 100,000 patients at 40 sites across the state. According to the Health Resources and Services Administration, Uniform Data System, 2016 data, 85% of patients were low-income, below 200% of the Federal Poverty Level. 18% of CHC patients are a racial or ethnic minority, the majority identifying as American Indian. Health centers served more than 4,000 migrant and seasonal agricultural workers and their dependents, almost 6,000 people experiencing homelessness, and 4,600 Veterans. All Community Health Centers participate as part of the Cover Montana Community. Additionally, the partnership includes other health care stakeholders like large health systems, Critical Access Hospitals, reproductive health clinics, Indian Health Service, Tribal Health Departments, and Urban Indian Health Centers. Since Cover Montana was established in 2014 and the project hosted its first VISTA in 2015, the project has supported this diverse group of partners in better connecting their communities to new health insurance coverage options. While insurance coverage and access to health care remains vital to Montana communities, health outcomes are impacted by much more. Having access to health care is vitally important, but health care alone doesn’t determine if someone will live a healthy life. According to a 2002 study by McGinnis Et. Al. published in Health Affairs, access to medical care was responsible for only 10%–15% of preventable mortality in the U.S. When someone is unable to get to their doctor’s appointment because they don’t have transportation, can’t afford enough to eat, or live in an unsafe housing situation, their health is impacted. These stressors that impact an individual or community’s ability to thrive are called Social Determinants of Health. A root cause of the Social Determinants of Health is poverty.

Member Duties : The VISTA will increase access to healthcare by facilitating the expansion of Cover Montana’s scope to include Social Determinants of Health (SDOH) and enabling services. The VISTA will provide education and outreach to more than 40 Cover Montana partners on SDOH to improve the efficiency and efficacy of their programs. The VISTA will build upon Cover Montana’s successful model to include targeted enabling services identified through assessment, then create meaningful outreach and education campaigns. Finally, The VISTA will evaluate the success of Cover Montana expanding to include enabling services information and assist in identifying and applying for funding that will allow long-term sustainability of the Cover Montana project.
