2018 City of San Antonio Community Diabetes Program
 
 
 
The Community Diabetes Project is on a mission to fight the diabetes epidemic in San Antonio, Texas. The City of San Antonio Metropolitan Health District (Metro Health) works collaboratively with the local YMCA to offer three free programs for either proper diabetes management or diabetes prevention. San Antonio and Bexar County, Texas experiences a high death and disease burden related to chronic illnesses, with significant health disparities in diabetes and its complications. To address diabetes and chronic conditions, Metro Health implements the Diabetes Self-Management Program (DSMP), an evidence-based program developed at Stanford University. DSMP empowers individuals to take control of their health. Through a series of six weekly sessions, participants gain confidence in their ability to safely manage the symptoms of diabetes to live healthy and fulfilling lives. Metro Health aims to enroll at least 1,200 individuals in DSMP each year. To achieve this goal, Metro Health must establish and maintain strong partnerships with various organizations and community-based entities for recruitment of participants and for workshop locations. Additionally, continuous improvement of program processes will be instrumental in evaluation efforts and to provide quality services to the program participants.

Member Duties : 1. Improve collaboration with City of San Antonio (CoSA)programs by planning and implementing outreach to recruit sites to host Self-Management Programs. 2. Identify best practices for planning, communicating, and scheduling workshops at CoSA supported program sites. 3. Develop process for follow up with partners who refer patients to the Self-Management Program. 4. Develop and analyze custom data reports to illustrate the impact of the Community Diabetes Project. 5. Assist with outreach and recruiting participants for Self-Management workshops. 6. Design and implement a participant feedback survey for the Self-Management Program workshops. Analyze results and present recommendations for improved operations. 7. Develop and analyze custom data reports from the results of the Self-Management Program Self-efficacy Survey and present recommendations for program improvement. 8. Develop process for follow up with program participants who do not complete the Self-Management Program.
