A Youth Homelessness Re-Entry VISTA
 
 
 
The Colorado Division of Youth Services (DYS) serves youth ages 12 to 21 who are either detained or committed. Following commitment, youth serve a term of parole at which time the teams are working to provide services and resources and ensure a sustainable plan is in place to support long term success. Areas of focus will include housing, resources, inter-agency coordination, training and transition planning. This position will support the central office and multi-disciplinary teams regarding re-entry of these youth as they return to their communities. The VISTA will work specifically with the Client Services Coordinator as well as the DYS transition task force and regional offices to put processes in place to ensure adequate resources and housing options. VISTA will also support collaboration with other state agencies as well as community providers to ensure continuity of care and effective service provision for youth and families.

Member Duties : (1) Identify local resources/providers and develop a resource guide/database in collaboration with the Savio Management Group (2) Develop application and tracking process for state housing vouchers to track how many vouchers are available and who has vouchers and when vouchers are released (3) Identify other housing options in local communities for youth exiting DYS (4) Coordinate with other state agencies and local providers to ensure effective collaboration (5) Provide consultation and training to DYS staff regarding transition and housing options (6) Regular meeting attendance with DYS work units to ensure effective communication and collaboration (7) Work with the Transition Task Force to support DYS transition efforts to support youth success (8) Other sponsor or VISTA identified project
