The University of Chicago Center for HIV Elimination (CCHE) seeks to eliminate new HIV transmission events over the next 30 years (from 2011 to 2041) by using network science to target and integrate prevention as well as create structural, person-centered, and community-specific interventions that reduce the opportunity for HIV transmission amongst individuals that are most vulnerable to HIV/AIDS. Our focus is to improve the overall health and well-being of patients in the program by addressing a wide range of medical and social needs. The CCHE program builds off of the medical home model of care and clinic without borders strategies, which offers patients that are the most vulnerable to HIV/AIDS a non-traditional response to their health care needs. CCHE patients are regularly asked about a range of unmet social needs and given access to a resource coordinator, community health worker, and community-based arts and culture programming. This posting is for positions located at the University of Chicago Center for HIV Elimination in Hyde Park. We have two full-time position starting this Fall in October/November. Additional positions may be available for a January start. Member Duties : C4P AmeriCorps members who are based at the University of Chicago Center for HIV Elimination (CCHE), will: 1) Recruit LGBT STI screening, education, and linkage to care service recipients of diverse ages who are most vulnerable to HIV/AIDS; 2) Help evaluate the effects of the CCHE services and conduct service follow up via social media, phone call, and/or text message as needed; 3) Leverage their networks and that of other volunteers to recruit participants and evaluate the CCHE program; 4) Directly serve these patients as care navigators and through the implementation of community-based arts and culture programming.
