The goal of this project is to increase access to healthcare services to homeless and housing insecure individuals in Flathead County through coordinating access to services to mitigate the health disparities caused by the social determinants of health. By targeting homeless individuals to assess/address some of the social determinants of health, and partnering with agencies that specialize in public assistance programs, Flathead City-County Department seeks to expand their scope of understanding on health in the Flathead Valley and address health disparities facing low-income individuals. Located in beautiful Northwest Montana, Kalispell and the Flathead Valley offer easy access to Glacier National park, miles of exceptional publicly-accessible trail systems, and superb outdoor recreational activities including hiking, fishing, biking, and skiing.

Member Duties : The VISTA will conduct research to improve the City County Health Department’s capacity to understand the foundational issues to improving health through assessing what barriers exist within the homeless population. The VISTA will help create communication strategies that build upon the tool FlatheadForward.com to ensure that agencies and coalitions are able to communicate and coordinate effectively for shared missions around improving the social determinants of health. The VISTA will expand partnerships with non-traditional partners to agencies that impact social determinants of health. The VISTA will help improve health messaging through standardizing social media outreach, website updates and messaging on the Flathead Forward tool, including housing insecurity and Veterans’ issues as identified by the Veterans Coalition.
