Cancer was the leading cause of death in Montana from 2011-2015, followed closely by heart disease, with a total of 5,763 Montanans diagnosed with cancer each year between 2011-2015. Low income Community Health Center patients face structural disparities in multiple preventative disease screening processes. Patient Navigation is a model of care that aims to reduce an existing health disparity. A patient navigator assists a patient in navigating the health care continuum to receive their preventative disease screening and chronic disease management in a timely and efficient way. This VISTA project will increase access to healthcare, including preventative disease screening, ensuring that households do not sink further into poverty due to a diagnosis of cancer.

Member Duties : The VISTA will increase access to preventative health care by investigating innovative ways to fund and evaluate a Patient Navigation project in Montana Community Health Centers. The VISTA will assess the current state of patient navigation in Montana by interviewing Community Health Center staff and community partners about current preventative disease screening practices. The VISTA will then conduct research on other states’ successful use of Patient Navigation programs as well as reports, evaluations and studies on Patient Navigation completed across the country. With this research the VISTA will develop a strategic plan using what they found from the current state of patient navigation in Montana and best practices from other states and systems. The VISTA will work with MPCA staff to identify and apply for grant funding that will allow for the long-term sustainability and capacity of a patient navigation project.
