2023-24 Healthcare Outreach Specialist
 
 
 
CCCTX provides a variety of client services to the community. The mission of the St. Gabriel’s Pregnancy and Parenting Program (SGPPP) is to empower parents to foster healthy relationships and create a supportive environment for their children to ensure a healthy and stable beginning to life. We provide SGPPP services without cost to mothers and fathers expecting a child or with children aged 0-3 years old. The Healthcare Outreach Specialist will play a pivotal role in assisting families with Medicaid enrollment and helping understand insurance benefits in order to maximize their benefits. This position requires an interest in public health, healthcare, community outreach and working with families with young children.

Member Duties : The Healthcare Outreach Specialist will work with the various Medicaid providers to understand what each plan has to offer then work with the clients to review their Medicaid benefits. They will provide healthcare case management and advocacy for the SGPPP clients. In addition, they will gain an understanding of the medical providers within the counties we serve and help SGPPP clients navigate access to a medical home. A portion of this position will require outreach to healthcare providers and organizing special health fairs and healthcare topics for SGPPP clients. This position is a part of Catholic Charities USA’s Healthcare Outreach Program, a national intermediary project. As a member, you will meet virtually with other members serving throughout the country, all focused on assisting individuals within healthcare outreach.
